What is Form SSA-44 (Medicare IRMAA life-changing event)?

The SSA-44 form, also known as the Social Security form SSA-44, is an official document issued by the Social Security Administration (SSA) and used to request a reduction in Medicare premiums (IRMAA) for Part B (medical insurance) and Part D (prescription drug coverage) under a qualifying life-changing event.

The full name of the form is Medicare Income-Related Monthly Adjustment Amount – Life-Changing Event. Its clearance number is OMB No. 0960-0784.


What is IRMAA and why file SSA-44?

IRMAA (often written with a mistake “IRRMA”) is short for Income-Related Monthly Adjustment Amount. It’s a surcharge that the government applies to Medicare Part B and Part D premiums.

The surcharge does not apply to most Medicare beneficiaries. It applies solely to individuals with high annual incomes. The amount of surcharge is calculated based on Modified Adjusted Gross Income (MAGI) from the tax return two years prior to the current Medicare coverage year.

Why file the SSA-44 form? Since the government uses 2-year-old data to calculate them, IRMAA surcharges do not consider the current financial situation of a filer. Over the 2-year period, you could’ve experienced a life-changing event that led to a significant drop in your income. If that’s true, the surcharge applied to you might be more than your current income can support. In this case, the SSA-44 form will encourage the government to consider fresh income data and reduce your surcharge.


Who can use the form: income drop plus a recent life-changing event

To be eligible to use the IRMAA life-changing event form, filers must meet two mandatory criteria:

  1. Receive an official notice from the SSA indicating that you must now pay a higher Medicare premium.

  2. Experience a life-changing event that is qualified by the Social Security Administration, and that caused a significant personal/household income drop.

Important: When filing the SSA-44 form, you will have to provide supporting evidence confirming your life-changing event and income decrease. You can report an income reduction that has already occurred or, in certain circumstances, an income reduction you anticipate this or next year. The form requires you to specify what income reductions have already occurred, as well as what reductions you expect in the following year.


Which life-changing events qualify and what the form contains

The form SSA-44 (life-changing event) is built around three essential elements:

  • A particular type of life-changing event

  • Reduction in income

  • Supporting documentation

The Social Security Administration only considers 8 events to qualify for the IRMAA reduction. All eight options are listed in the SSA-44 form for you to choose the event that you experienced:

The 5 steps (sections) of Form SSA-44

The SSA-44 form is standardized and always consists of the following 5 sections called Steps:

  • Step 1: Type of Life-Changing Event. This section provides a list of 8 qualifying events with checkboxes. It also includes a separate field for indicating the date(s) when the event occurred.

  • Step 2: Reductions in Income that have Already Occurred. This section clarifies the details about the event-related income drop. Filers need to specify:

    • The tax year during which the income decreased

    • Adjusted gross income

    • Tax-exempt interest

    • Tax filing status

  • Step 3: Anticipated Reductions in Modified Adjusted Gross Income Next Year. This section is optional and should only be used if the filer expects their next year’s income to drop even more. If yes, the filer must specify:

    • The next tax year

    • Estimated adjusted gross income

    • Estimated tax-exempt interest

    • Expected tax filing status

  • Step 4: Documentation. This section of the form is non-fillable. It specifies what supporting evidence you need to attach to your form.

  • Step 5: Signature. This section provides general acknowledgment and consent statements that filers need to read before signing and gives dedicated space for them to provide their signatures, mailing addresses, and phone numbers.

Steps 1–3: choose your event and enter income details

Steps 1-3 are the most important ones in this document. Find detailed SSA-44 instructions for filling them out correctly below.

Step 1:

  1. Read the list of qualifying events.

  2. Check one event that corresponds to your situation.

  3. Indicate the month and year of the event. Keep in mind the general rule of thumb – the date of the event should precede the tax year when your income dropped.

Step 2:

  1. Specify the tax year when you filed taxes with a decreased gross income.

  2. Type the adjusted gross income (find it in line 11 of the IRS 1040 form for the respective tax year).

  3. Indicate tax-exempt interest (line 2a of the 1040 form).

  4. Choose your tax filing status for the indicated year.

Step 3:

  1. Check one of the two boxes (Yes or No) depending on whether you expect your gross income to be lower in the following year.

  2. If you choose No, proceed to the next Step.

  3. If you choose Yes, specify:

    1. The year

    2. Estimated AGI

    3. Estimated tax-exempt interest

    4. Expected filing status

Step 4: which documents (evidence) you need

According to SSA-44 documentation requirements, every filer must provide two types of supporting evidence:

  1. Proof of adjusted MAGI

  2. Proof of the declared life-changing event

To confirm the difference in your gross income, you will most likely provide your annual income tax return (the IRS 1040 form). The documents needed to confirm the life-changing event vary depending on the specific event type:

  • Marriage: A marriage certificate.

  • Divorce/annulment: A divorce decree or annulment document.

  • Death of spouse: A death certificate.

  • Work stoppage: A retirement letter or last pay stub indicating retirement.

  • Work reduction: A last pay stub or a formal letter from an employer indicating reduced hours.

  • Loss of income-producing property: Insurance documents, court documents, or other official records of a disaster, fraud, theft, etc.

  • Loss of pension income: An official letter from a pension fund indicating the termination or reduction of pension.

  • Employer settlement payment: An official employer letter indicating the terms and impact of the settlement.

For all documents, you can use either originals or certified copies. You can either attach them to your form (and have them mailed back to you) or show them to an SSA employee.


Which form year and tax year to use

If you look at the upper left corner of the SSA-44, you will notice the month and date of the form. The Social Security Administration periodically updates the form (typically when its OMB clearance is renewed) and discontinues prior editions. This is done to have the form reflect the current Medicare income thresholds and premium amounts. The structure of the form itself stays the same, though: the SSA-44 for 2025 and 2026 uses identical steps and fields, so only the edition date changes. The latest edition is labeled 12-2025.

To fill in the tax year fields in Steps 2 and 3, follow these tips:

  1. Check the IRMAA notice letter to see which tax year the SSA used to estimate your MAGI.

  2. In Step 2, choose a tax year that is more recent than the year of the used tax return information. You can also choose the premium year (the year when you received the IRMAA notice) if your income was not reduced until then.

  3. If you are filling out Step 3, indicate the following year after the tax year specified in Step 2.


How to sign Form SSA-44

Before you can file the SSA-44 form, you need to sign it. Our editor lets you do this quickly and fully online:

After you complete the SSA-44 form and collect the necessary documents, there are 3 ways to file your request with the SSA:

You can submit your SSA-44 online through the official SSA website. To do this, you need to sign in to your account (or create one) and upload the form with evidence.

The online application should suit the majority of filers. However, you can file your form in person if:

  • You want to protect sensitive original documents.

In this case, you need to look up SSA offices nearby and arrive at one of them with your documents.


Where to mail Form SSA-44

There is no single national mailing address for the SSA. So if you decide to send an SSA-44 form via mail, you need to find an SSA Field Office located nearby:

  1. Go to the SSA’s Field Office Locator page.

  2. Type your address, city, state, or ZIP.

  3. Pick an office that suits you and copy its address.

  4. Gather your SSA-44 form (our form is printable, so you can print the signed copy for mailing) and supporting documents, and mail them to the selected office.


How long does SSA-44 take to process

It’s hard to predict the exact SSA-44 processing time because it depends on the office you apply with, the individual handling your request, and current workloads. SSA publishes no official processing-time estimate for SSA-44. Unofficial estimates range from a few weeks to a few months, depending on individual factors.

You can monitor the status of your form via your account on the SSA Application Status portal or by reaching the SSA at 1-800-772-1213. If your form is approved, you will have your premiums recalculated and receive a refund for any overpaid amounts in the form of a check or credits toward future payments.

If the SSA recalculates your premiums incorrectly, you can gather additional evidence and file an appeal (the same SSA-44 form with added evidence).
