What is the W-9 form used for?

When might filling out a W-9 not be necessary?

Instructions for filling out the W‑9 form:
Line 1
Name – Enter your full name, ensuring it is spelled correctly and matches the spelling on other tax forms you’ve completed.
Line 2
Business Name – Complete this field only if you own a business and use a name (trade name, DBA, or entity name) different from the one in Line 1.
Line 3
Tax Classification – Indicate your tax status according to IRS classification. Depending on whether you’re completing the form as an individual, corporation, LLC, partnership, or trust/estate, check the respective box. Only one box should be selected.
Line 4
Exemptions – If your entity is exempt from backup withholding, provide the appropriate code in the first line. If exempt from reporting under the Foreign Account Tax Compliance Act (FATCA), enter the relevant code in the second line. Note: Individuals typically do not complete this section.
Line 5
Address – Enter your street address (including apartment number) so your employer can mail you information returns and notifications.
Line 6
State, City, and ZIP Code – Provide your city, state, and ZIP code.
Line 7
Account Numbers – You may list any account numbers your employer might need here. If none, leave this section blank.
Personal Details Section
The second part of the form requires your personal details for your employer’s tax reporting. Fill out this section as follows:
Part 1
Taxpayer Identification Number (TIN)
Provide your Social Security Number (SSN) if you’re completing the form as an individual or single-member LLC, or your Employer Identification Number (EIN) if completing it as a corporation or partnership. Sole proprietors may use either number. Resident aliens without an SSN can use their TIN.
